    they must have signed something in store, or another family member has requested it. they don't just send out cats willy nilly

    It happened to my wife. She phoned up and asked how she could be sent it without requesting one. Something to do with instore questionaire IIRC - will ask her later about it.

    If they ever ask you in the store if you want to enter the prize draw, unless you tick a little box to say that you dont want a book, they will send you one. It is on the form to say this but like most things people dont normally bother to read things when they are in a rush.
    I'm sure that they will get someone to collect it and refund the money
