This is not a criticism but I am guessing that not having a garden means she has limited opportunities to let of steam, even if only for a minute or two.
The behaviour on the lead is most likely frustration and lack of self control. As is the barking.
Do you clicker train?
A method that I use a lot is Leslie McDevitt's 'Look at that ' game.
Traditionally, we punish or correct our dogs for reacting to a stimuli such as dogs, people, joggers, traffic. All this usually achieves is at best to supress the behaviour but still leave the dog feeling frustrated or at worst to make the problem worse.
The look at that game teaches a dog to look at but not react.
I was looking to see if there was an explanation on line anywhere and came across these two videos which I think demonstrates the technique quiet well.

The other stuff I mentioned before really does help in terms of helping a dog to relax.

When i got pregnant and couldnt hold lex on the lead with his pulling. i got a halti omg what a diffrents.
The funny thing is if he is off the lead he walks to heel mostly.
