    With the gagging thing on the boil em up type have you tried cuttin away some of the back of it? When i was a kid they boil type were massive and they used to make me gag too. They used to go all the way to the back of my rear molar. My dad cut away a few mm which meant it covered all except the back molar which gave more than enough support to the rest of my teeth and solved the gag reflex issue! Maybe try it out on one you already have moulded and it'll cost you nothing.

    As for getting one that stays in with fixed braces - good luck! I had trouble just getting it to stay in on my teeth but thats why you're meant to have your mouth closed most of the time and breathe through your nose! Mine was for Karate though so my mouth was generally closed when i got punched in the chops
