Yea you do. I phoned the inland revenue to get the address as there website is pretty terrible for finding information on. Sorry, I can't remember what the address is now.
I believe it will depend on who your employer is - certainly if you are still working it is the office which deals with your employer, and your payroll / HR people should be able to tell you who this is - or there will be a reference for it on your P60. If you're not working any more then the office nearest to where you live should be able to help.

Don't forget that if you have any money in a savings account and you are not expecting to earn more than the personal allowance this year then you can fill in an R85 form (which your bank / building society should have easily available and know what you're talking about, although they sometimes run out!) and get your interest paid gross. If you realise part-way through the year that you're going to earn over the £5000 then you should tell your bank / building society, and the tax office.

I filled out a P50 form from the inland revenue website and sent it in to them a few weeks ago. I tried to ring my local tax office first, but it was constantly engaged..... I haven't heard back from them yet.... Any idea on what the time scale is for payment or whether I've done completely the wrong thing?!

I did one for 2004/2005 and I think it took around 6 weeks.
