They didn't check my bank statements. I just filled in the form as my income from student loan at my student address. It's definitely worth having this form for Dentists, prescriptions, eye tests etc.0

No, as paddy3d said, you just fill in the form with your income details and send it off - you don't need to supply supporting evidence or anything. I was a bit confused by the form at a couple of stages, so I phoned up the helpline and they were so helpful - the bloke went through the entire form with me so I knew it was filled in correctly, and my certificate arrived a couple of weeks after thatoh so they check your bank statements?

i've checked and basically i'm not pregnant and i'm over 18 (20), so i guess that means i'm entitled to nothing? all it is is that i am in full time education, i get no income support or EMA or grant from the SLC or my university as my parents earn 50k combined.
would i still be entitled to partial help even if i get no support from the government currently?
also i go to uni in wales (Cardiff), where prescriptions are free... is dental care free too?

I'm the same as DomTom in all but the EMA - I got it, but the HC1 form doesn't take into account parental income; it is JUST the applicant's income, so if they are in full time education and earning below the threshold for paying tax, then they are more than likely to qualify for at least partial help. I qualified ror full help, so methinks DomTom has a good chanceYou could try but more than liekly will be rejected.

Hi,
If you are 18-19 and in full time education precriptions/dentist/optician are free. After that get the HC1 form filled in to get a HC2 form. I think the savings threshold is actually £8000 and they do count student loans & grants as part of your income (one year at uni my student loan and NHS busary were over their threshold even though I really didn't have any money to actually go out and spend so I wasn't entitled!).

okay see im on a year in industry, earning money this year only. but not going to lie, i don't want to spend it on a dentist when i've been working my !!!! off to pay bills during my first 2 years of uni. i'm still confused as to what proof they needed for the money you had. if its the previous tax year earning thats fine... if i have to show them my statements where my payslips go.... well..0

I think check-ups are free in Wales until age 25, but you have to pay for treatment.0
