Mortgage Interest and Mi 12 form

Hello,
Can anyone help me, I am now in the process of resending my 3rd Mi 12 form back to my lender for them to complete and forward onto the address stated on the pre paid envelope DWP.
Either I have been unlucky with both previous forms getting lost (forms have been sent on from my lender successfully) but seem to disappear when they need to get processed by the Tax office.
Has anybody got a direct contact number for the housing claims dept/mortgage interest claims dept or any experiences of delays/excuses not receiving the forms?

If you are sending the form to the tax office it definitely will get lost - the tax office do not deal with SMI!
The SMI Team will be based at the Processing Centre for your area. This number should be on some correspondence regarding your benefits.Gone ... or have I?0 -
dmg24 is right. You need to contact the Mortgage Section at your local benefit delivery centre.
The forms come into their office, are sorted into benefit type (IS, ESA or JSA) then go into the queue for processing. And it's a helluva queue ... so give them a ring, get someone to check for the form & find out if they need any further information to support your application ... and establish when their help is going to kick in.0 -
Daza747! I'm currently waiting on a MI12 form being sent out, and as others have said it the Benefits Agency that deals with other claims that this gets sent back too. Also you need to be in receipt or JSA or IS for at least 14weeks before payments would start.
