GP wants to charge excessive fee for Medical Form

Out of interest and obviously without disclosing any personal medical info, could the OP say what the 4 questions on the form are?
I do understand everything people have been saying. I do feel though that something should be in place to help students.
On the one hand we're giving a lot of them £30 a week EMA to attend college to help encourage them onto further education, then those that go on to Uni are not only leaving thousands of pounds in debt, but those needing such things as the OP needs is going to have to spend probably a weeks living money just for that one form.
OK some students have parents that can help them, but a lot don't.Martin Lewis is always giving us advice on how to force companies to do things.
How about giving us advice on how to remove ourselves from any part of MoneySupermarket.com

In my experience, my GP charges me when he thinks I can get the money back, e.g. if my employer has requested a note. If it is a cost I am likely to suffer myself, there isn't a fee.
There don't seem to be any fixed rules.
