As I read it if you are on PAYG you'll pay the standard UK call cost per minute whatever that might be, with their cost on top.0

You can if you use their 020 access number but your pay your payg rate to the 020 access number + 18185 4p connection plus 1p a minute.
Get a geographical access number that you can point direct at USA Number from someone like http://www.localphone.com/call/united_states calls will only cost 0.7p a minute no connection + of course your payg rate to call the number.
If you have an unlocked mobile you could get a http://www.lebara-mobile.co.uk/en/ payg sim and call USA direct for only 4p a minute.0 -
