I agree for the work issue the best option is honesty. Explain you have been having problems (maybe even mention you have had some problems outside of work as well). Tell them you have felt under a lot of pressure, even say you feel relieved not to be dealing with the account again.
If people acknowlegde their mistakes and agree to work to improve things going forward its much easier to be forgiving than if someone refuses to admit they have done anything wrong.
