can this be a relative, or am i better getting my occupational health worker to do it? or does it not matter? my mum is with me most days to help so she knows best but i am not sure if its the best one to put

your mum would be fine - there is also nothing stopping getting additional statement from OHW as this may also be helpful. just make sure additional pages have your name etc on them.
you ask as many questions as many times as you like - no-one is forced at gunpoint to read posts and i have found most people on here are SO helpful.0 -

Well, i have just typed a letter to GP/rhumatologist and OHW stating i am applying for DLA after reading literature they provided me with. i have listed all the equipment the OH has provided me with and a brief day diary that shows what i need help and assistance with, and obviously asking for their support in my application0 -
I agree use a statement from your mum she will be able to go into more detail, my son does mine cause he lives with me and knows me best.
I sent my renewal form in end of july, so will wait and see what happens.

My boyfriend wrote mine for me as he sees me most days. It was a little bit upsetting to see what he wrote (all of it true mind)..never realised how much picks up on. Posted it off today.

ditto, its hard to see in black and white how much they do beyond the ordinary for you0 -
My work colleague and neighbour did mine, as he sees me at both home and work. But getting a letter from your OHW as well as someone who knows you personally and sees you everyday as well is a good idea. Good luck!0

thanks all, my mum has done it and i cried at what she had wrote...as if filling the forms in myself wasnt bad enough..but to see another persons perspective is another thing all together.0

yes hun - it IS hard when you see what others write- but sometimes it can be helpful and reminds us to be grateful! we often take our helpers for granted and a wake-up call now and again is useful. cheer up now and think of the things you CAN do.
