Halifax Subject Access request

Hi there just a quicky really. I wrote to Halifax on the 29th April 2009 asking for the data they held on me including the fee of £10.00 on the 13th May they replied saying they had recieved the fee and were processing the request.
On 24th July 2009 I wrote them a reminder that they had gone over the 40 days allowance and I would give them a further 7 days to get the information to me.
I then recieved a letter dated 11th August 2009 saying they did not have sufficient information to confirm all the details required and were unable to pocess the request they asked in the letter for:
Full address details inclusing addresses if I had not lived there for over 3 years (which I have and did include in the letter my full address)
The correct payment of £10.00 (which I did include in the first letter and they sent me no information- so I shouldnt need to include another cheque should I?)
also a copy of my original request.
Im stuck as to what to do next? This is all in response to Capquest contacting me saying they were collecting debt from Halifax of nearly £1000 so we had copies of statement through the post and it is all credit card charges but I paid them off in full but they continued to charge me, but I need all the evidence including transcriptions of telephone calls etc. I got the credit card early 2001 and paid it off 2004. But they added charges since then.
2004 I spoke to a guy on the phone who took my payment and said that my account would be shut down, I thought nothing of it until 11th July 2008 when Capquest soliciters H L Legal sent me a letter.
Sorry if this post is all over the place but I am in bits thinking about it please help.

Capquest have sent me statements etc from Halifax but I would like copies of the records they hold i,e notes that were put on my account the people that I had spoke to even transcriptions of conversations as the guy I spoke to said if I paid my account balance it would be closed down, but it didnt and they carried on with charges ontop of charges:A0

Am I right in saying CapQuest are demanding proof that you paid in full the amount?0 -
They keep demanding payment, they were phoning up to 7 times a day, until I had a panic attack at work and my mother in law phoned them and said unless they wanted a court case of harassment they was to stop, which they have. I have been updating Capquest on the dealings that have been going on and they have responded in a polite manner saying to keep them updated. Looking through the statements the final one before Capquest bought the debt says, " Principle abandoned £298.45 Interest Abandoned £101.09 Insurance Abandoned 15.14 Other fees abandoned £578.19" What does Principle abandoned mean? I have googled it and nothing has come up. Would I be best claiming the charges back off Halifax and then paying the money I recieve to Capquest, I went to see CAB and they wasnt very helpful they said contact Halifax if you dont get a response then you will have to pay

Hang on, what does INSURANCE mean, did you have PPI?
I would get Halifax to provide a copy of that as well0 -
From my first statement dated 5 March 2001 to statement dated 4 August 2004 I have been charged Repayment cover ranging between £2.05 - £5.59 the repayment cover stopped 4 months after I though my account had been closed.:A0
