I can recommend plus.net. I've been using it since October and have had no problems at all.
1GB data transfer @ a speed of 1MB for £14.99 per month. If you go over the 1GB they just charge you extra (I think about £2 per 1GB zone that you use). If you constantly use more you can upgrade your account and I think that is free but not 100% sure about that.
You can move to plus.net from another provider for free, don't need to pay an activation fee.
You can buy a modem through them or not at all if you already have one.
There is a one off activation fee, price depending on whether you take out a monthly or annual contract (£70.50, £58.75)
They have dial-up and pay as you go options. (I'm on a contract so dont know how these compare)
