I disagree. Systems should be in place to stop this happening and if its not reported then the factory can not stop it happening again. It may well be that they aren't aware of a fault and an investigation would highlight something that needs fixing.0 -

I am sorry but that is rubbish!
Food manufacturers should have a robust HACCP system in place to prevent this.
If not they are breaking the very strict hygiene rules they profess to work to.0 -
