Graham, can I just ask roughly when it was you applied? My car arrived a week or so ago, but I've not received any information on Diamond Club membership. I've used the card a few times since it arrived.0

I'm sorry to be a bit vague, but I applied just after the 2nd MSE newsletter that mentioned this offer - I'd say around the 5th April , and the card / Pin took about a week from then to arrive.
Here's the sequence of what I recieved :-
PIN letter
Card
Diamond club membership letter (Dated 13th April)
Miles confirmation (Dated 3rd May)
