Early mobile contract cancellation, sample letter?

I wish to cancel my mobile phone contract 3 months early and was wondering if anyone had a sample letter that I could use to write to the phone company?

What's the reason for cancellation?

You can cancel an account anytime and some providers will take cancellations over the phone - but you will still need to pay the line rental due up to the end date of the contract.
