Cash back about Dial-a-Phone?

I got a phone with O2 from dial-a-phone with 8month free line rental cash back . But in there website, about how to claim my cash back, it just said: 'You will receive full instructions on how to claim your entitlement in the paperwork with your welcome pack.' and 'Post your most recent bill within the qualifying period to the following address.' And in the welcome pack, it just mention send my bills to their company but there is not anything about a 'voucher'. (Because other websites need a voucher to claim cash back)
So I want to ask anybody has ever bought phone in Dial-a-Phone? And how should I claim my cash back? DO I need a voucher?
Thanks very much.

i had a dial a phone contract last june with cash back, and the cash was deposited in my bank without me having to do anything. Dont know if this still applies though.

I have phone with Dial-a-phone. I just send my bill into them, once they recieve it they text me. Once they paid the money into phone account and the second time they sent me a cheque. Have never had any vouchers

Have had three Dialaphone contracts all with 3 and each time have had to return a voucher with the stated bill. Each voucher would state how much the cashback was for. They always paid me by BACS transfer within two weeks of submitting the voucher.
How and when you get paid depends on which Network you are with as they DO differ.

I didn't receive any vouchers with the 3 network contracts I've got, so I rang them up and clarified it over the phone.

I was told I had to send the front page of the bill to them.

I did have a few probs but nothing that couldn't be sorted.

I have not had any problems, no vouchers, had to send the original bills with a covering letter, cashback credited into the mobile account. I recommend them wholeheartedly.
