OSPS redemption hassles

I got a k750 from them in june last year cashback on months 4,8 and 12...I was originally misguided by a member of cpw staff and sent my first 2 pages of my bill to cpw...after calls and emails this was resolved in my favour.

when I called osps today to confirm the cashback address I heard talk of forms? on the automated line... when I told the assistant I had no forms she replied I had to inform osps of this within 14 days of my contract starting.

I am due £55 cashback and have 3 1/2 months to run on my contract...where do I stand here?

the contract I got last week had an A5 piece of paper in amongst the paperwork with 3 'forms' to fill in when I was claiming the cashback elements of the deal. On other paperwork it said if I hadn't received that form to contact them within 14 days for a new copy
