So if I refunder her in full through paypal could she then open a dispute - surely it would show that I had refunded her?

Nope, she can only dispute a payment that has not been refunded as long as you refund the original payment received. Therefore sending cheques to the hospice won't work unfortunately as the original payment would still be able to be chargebacked.
Personally I would stick to your original idea, call her bluff, tell her to send the item back unopened and you will refund in full. You should not paid her back, if she opened dispute, she needs to return the goods to you before getting return. Don't let her have both money and chocolate!!!0 -
Still had no reply from her - I think I would rather she open a dispute now because it's easier than all this emailing etc.. and waiting for reply.0

I would leave it now and wait for her to make another move.
