they want my signature???

i have sent the template letter available with a cheque to lloyds bank who i have a credit card debt with for around £6500. they have written to me today with this long form attached that they want completed before they continue with my request. most of the things they are asking is ridiculous like my name and address but they also want to know things a little more difficult, such as they want me to list specific dates and times of all the phone calls that i require copies of??? how do i do that?? they have whole sections dedicated to me explaining exactly what information i want specifically and why??? but the most glaring thing they have asked for is me to not just sign the form i will be completing but also to provide a copy of my signature in a box which has me saying i am requesting bla bla bla but the box is the same as they type you sign when you apply for a loan or credit card where they want my name under account holder and then signature in signature box and then to date it?? the form itself is white yet they have shaded this section in grey??? do i have to provide this for them to complete my request as they have said??? maybe i am being paranoid but something about the way it has been requested and in that particular format just makes me feel a little suspicious ???
anyone have any thoughts or advice on this???

    "Firstly, the data controller must satisfy himself that the person making the request is, in fact, the data subject. The use of a subject access request form is advised, since the greatest breach of a data controller's security is for the data controller to satisfy a subject access request made by a person impersonating the data subject. The use of the form goes towards proving that the data controller has adequate identification and verification procedures in place.

    Secondly, the data controller is entitled to ask the data subject for further information to enable the data controller to locate the information which that person seeks. It should be remembered that the individual is within their rights to request copies of all information held about them. However, you may wish to try to help the data subject to try to narrow their request for information. This is discussed further below."

    If you have asked for information about you, it sounds reasonable for them to ask for your signature for proof that you are who you say you are. You'd be hopping mad if they gave that info to a third party without even checking the signature.

    If you have asked for all information they hold about you, then I think they are in their rights to ask you to be more specific. They can't be expected to check through every single phone call just in case it's yours going back years. You need to give them more information on what exactly you want.

    i'd still be suspicious of the fact they want a signiture.

    My request for a true copy of my credit agreement under section 77/78** was made on xx/xx/2008 and the 12 working days for your compliance expire on xx/xx/2008.

    I note that there is no provision that removes the requirements of the act to provide this information on time, even if you are unsure of my identity.

    This sounds like you've issued an SAR request, if so, it's the ONLY time you're required to sign the letter, when you sign, enter a couple of XX across your signature.

    thanks to everyone coming on, this is a copy of the letter i sent asking for the sar, i dont see really what they want me to clarify???? can anyone see something i cant maybe??? as for the telephone calls i do not know myself exactly which ones i want? it was just part of the template. as for the signature bit i dont know what to do now as there seems to be a difference of opinion? or i could be reading it wrong?? i have been married 3 times since the original signature on application so i dont know which if any they still have. the original was signed when i was 16 and am now 45

    In the first part under the Data Protection Act 1998:

    Under the Data Protection Act 1984 and 1998, and including the right of subject access under these acts, please supply me with a complete list of transactions and charges relating to the above accounts held with you since they were opened.

    Additionally where there has been any event in my account history over this period which has required manual intervention by any member of your staff or any other person, I require disclosure of any indication or notes which have either caused or resulted in that manual intervention or other evidence of that manual intervention in relation to my business with you.

    If you are unable to supply data relating to manual intervention because there has been no such manual intervention then please be kind as to confirm this in your response to this request.


    For the avoidance of any and all doubt, I reiterate:

    I hereby request that you supply me with any and all historical data in your possession which, in any way appertains to me, including (but not exhaustively) a copy of the original signed executed agreement; statements of account; duplicate statements and/or print outs of all account transactions; all internal and external correspondence sent or received by you including memo’s, logs, notes, screen prints and transcripts; notes of manual interventions such as telephone attendants' notes, copies of stored telephone conversations, internal and external emails; any other information held on any/all types of media in any relevant filing system (microfiche included).

    If you have disclosed any information to a third party (with or without my express permission), will you please include details of this in your reply, along with notes of any legal action passed or pending (to include a true copy of default notices, court orders and the like).

    Where any information that you provide includes any charges, for example returned payments, late payment fees, and so forth, would you please advise your breakdown of actual costs (liquidated damages) incurred for each charge, and the Term or Condition on which you rely upon to claim such a charge.


    I also require that you forward, within the above mentioned time scale, a true copy of the Terms and Conditions that were in force at the time my account was opened, and any subsequent amendments to those Terms and Conditions.

    I enclose the statutory maximum fee of £10.00 to access ALL data held by Lloyds about myself. You should be fully aware of your statutory obligations under the Data Protection Act and that any failure to comply with this request will involve a complaint to the ICO as well as potential legal action.

    You have 40 days in which to comply with this request.

    This communication has been sent Recorded Delivery so I can ensure compliance on these issues within the legislative timeframes.
