the letter suggested in post 6 seems to cover it all
The letter in post 6, is totally different from an SAR request.

The letter in post 6 is for a CCA request ONLY, which does NOT require a signature.

An SAR DOES require a signature.

    How much did you pay?

Right, that amount is for an SAR, which does require your signature.

Personally, i wouldn't give them a copy of my signature as this is part of what i am trying to get from them.

    Data Protection Act s7(3)
    (3) A data controller is not obliged to comply with a request under this section unless he is supplied with such information as he may reasonably require in order to satisfy himself as to the identity of the person making the request and to locate the information which that person seeks.

If they have been quite happy to previously send account information to your address, they cannot say that they are unsure of your identity. In my opinion a signature is not required to prove your identity. A copy of your birth certificate and a utility bill would be enough to identify you and where you live.

The letter you sent them was very clear in what your requirements were so i can't see how filling in this form will make that any clearer. They have sent highly confidential information to you at this address already without asking for your signature - pin number and statements.

In my opinion the clock is ticking and the have less than 40 days to respond. The legislation does not state that a signature is required (as far as i can see) and they already have your payment and requirements.

If you were to put a few lines through your signature, it would be fairly easy to take these out with a graphice package. If you do intend to send in the form and sign it, change your signature slightly - possibly adding the first letter of the last line to the end of your signature so that you have a better chance of seeing if it belongs on that document or not.
