Putting gloss on top of gloss will NOT cause it to bubble, the only time gloss bubbles is when moisture gets into the gloss, and reacts with the oils.
A Primer is NOT only for bare wood, it is also used when timber has been treated incorrectly and the moisture is escaping. An undercoat will also let moisture through. Using a primer will seal the door and prevent any further moisture escaping.
TRUST me, I have spent enough time dealing with customers whose paint was either bubbling or flaking, and 99% of the time it was due to either no primer being used or the wrong type of primer.

Just because it is happening to your banister as well, doesn't mean the house has excess moisture, it simply means that it hasn't been treated properly either. It is quite possible that NONE of the woodwork has been treated properly, even if the rest shows no issues. I have been to properties where all woodwork was treated the same (i.e. without any primer) and only parts of it were problematic.

Moisture on the top surface of gloss wont bubble, but it will discolour more quickly...

    I give up....
If it looks like a duck, and quacks like a duck, we have at least to consider the possibility that we have a small aquatic bird of the family anatidae on our hands

It's nothing to do with the dampness in the bathroom, or ANYWHERE else in the house. The dampness is in the door, it always will be and no amount of drying/dehumidifing will change that. If you were to take all the dampness out of the door, it would warp. Did you actually read my posts? ALL YOU NEED TO DO IS USE A WOOD PRIMER BEFORE GLOSSING AND THAT WILL RETAIN THE MOISTURE IN THE DOOR - WHERE IT SHOULD BE!!!!!

Why ask for advice if you aren't going to follow it?
    nasty........
