Help needed Re. Attachment to earnings form received.

Hi, I'll keep it as short as poss.
I have a CCJ which is for £300 to pay forthwith,
I wrote to court on receipt of CCJ,sent my and made offer of payment.
Court wrote back saying I needed to pay a fee for redetermination, I didn't have the fee and so wrote to claimant asking to pay by installments.
I did not receive a reply, so i did not pay. I expected they would send bailiffs and I could try and sort it with them, but nothing happened.
Fast forward, I just received from court an application for attachment to earnings, Form N55,
I need to send it back asap.
The debt is mine, nothing to do with mu hubby.
Problem is do I just fill it in myself, I work part time and don't earn and it all goes out, hubby works full time, We have two kids.
I am unclear as to weather I include his income or not. It is not his debt.
Also has anyone any idea of what sort of expenses are allowed. I know not smoking, don't smoke anyway, No Sky TV.
Any tips on this would be great.
I really want this sorted on the form.
I DREAD THE THOUGHT OF THEM ORDERING ME TO COURT if I don't fill this form in correctly. The court is miles away too.

    I need to go back to the begining
    I replied to the claim form and offered to pay by installments.

    Did you send this to the court or claimant?
    Next thing that happened I got told by the court that I had to pay forthwith
    If the claimant rejects your offer, they win by default, and a CCJ is granted, what did you do when the court informed you to pay forthwith?

    Thought they would send bailiffs but nothing happened until last week, one year later.
    It may help people here to understand further if you said what the debt was actually for? Credit card/loan/utility bill and so on. And it's a little hard to read when you seem to type in different font colours.

    Excuse me but am I missing something because I have tried to understand your post but ... it's pretty confusing. And it's almost reading with all these different colours, emboldend letters like one of those letters from a DCA TBH. Except in reverse.

    Surely you must have had other paperwork in the year, as you say, you've waited. Something appears to be missing somewhere.

    No nothing is missing i have not had a letter, Phone call, nothing,
    My OP asks for any info regarding filling this form out as the debt is mine, not hubbies, as said in OP.
