It would seem that CD WOW! would be deemed to have accepted your application by sending that - as long as you haven't breached any other T&Cs (multiple applications per household, for example.)
In the event of any future possible dispute, it would be useful to have a copy of that page to hand.
Can I refer the MSE'ers on this page who are debating the rights and wrongs of getting or not getting the voucher to my previous post on p1 of this thread as shown.
Where CD-Wow may have "slipped up" is by sending that automated acceptance. If they wanted to check and vet the response it should really have been done at that stage - not now.
Personally I think it is rather an intrusion of privacy to start quizzing anyone now about your shopping habits.
As long as you haven't breached any other T&Cs, my view would be that if CD-Wow sent you that confirmation they have entered into a contract with you to supply the incentive voucher.0 -

There is a sensible point perhaps to be made about people here and on other BBs who abuse freebie offers with multiple applications - but yours is not the way to make it...and as I've said, the ground rules on this one are entirely different.0 -
When you signed up for the voucher it said that it would take 10 days. Those 10 days have not passed yet so what is everyone getting so worked up about?0

I emailed CDWow! earlier today and have just received a reply. They are asking me to confirm my address, which they already have, and they want to know where I bought a copy of Time Out.
It seems like they are looking for ways of getting out of this deal. If they don't sort it out it will end up being a major public relations disaster for them.
