    I'm really grateful for all the suggestions, as I use Open Office for everything else this looks good, and I've had a look at the other ideas too, thanks everyone - and wish me luck with the job!!

    Might sound a dumb question but are you sure that when you open it using Adobe Reader that you cant type into the boxes to provide the information? I have seen this on other website where the form is locked but you can type in the fields.
