Carphone warehouse redemption and online bill

I sent my 4th online bill to carphone warehouse(o2 contract) and today I got a letter from them saying they can't process the redemption as the online bill doesn't have name and address on it. I entered these details from on day one using "edit personnel details" link on their website. But it neve shows the name and address on the online bill. I called their helpline 08700870168 and got the instruction that if I enter these details using edit personnel detail it will appear on the bill. But for me this doesn't happen.

Can anyone please help how to get name and address on online bills.
