Difference between an soa and cfs ?????

Evening All, I have sent my 1st letter using the template on this site to Halifax claiming hardship, they have sent me back a form called a Common Financial Statment.
I have have seen plenty of threads about statement of affairs but this form is asking about case ref numbers and agency postcodes, i am thinking maybe they have sent me the wrong form?

They are essentially one and the same. The CFS is generally used by debt advisors (National Debtline, CAB etc) on behalf of their clients and is an industry-wide recognised format. (thus the case ref and agency postcode etc)

Thanks Orford, so i can fill it in, send it off and just put n/a for the agency and case number bit or would it be better to ask for a soa form?0
