skint - I'm totally confident I'm having a girl but all seven of my scans have been carried out by a consultant in feto-maternal medicine and the five scans I've had since I was 12 weeks have all shown that I'm carrying a girl.
Some sonographers can tell when scanning BUT because most women only have a scan at 12 weeks, 20 weeks and occasionally 34 weeks it's definitely not 100% reliable.
A co-worker was told at her 20 weeks scan that they thought it was a girl so she was a bit shocked when she gave birth to a boy 19 weeks later but that was nearly 8 years ago.
It also depends on the scanning unit - but my impression is that nowadays they don't say anything unless they are really confident.

I know of 2 who have said they were having girls, and girls popped out! One being my beautiful niece! Saying that though, even though I knew she was going to be a girl, I still bought neutral coloured things just in case!!
