The form you need is 11CTC, this can be found on the HRMC website. Not sure if I can post links on here, but here it is.

I have today sent my form off as when I phoned they were experiencing high levels of calls!

I certainly think it is worth a go, the form isn't that complicated particularly if you're a single parent, just one page.

hi, can anyone help me where to send this form to once filled in, should it be your local tax office, or the one where you wages were done at the time , or the child tax headoffice?? im a bit stuck sorry to be a pain x

Tax office that deal with your current or most recent employer.

Not always the office that's closest to where you live.
