what is IS10 form look like ? Is income support form different to IS10 ?
It's a 3 or 4 page questionnaire & the information you provide on it will be used to determine whether you qualify for the Severe Disability Premium part of Income Support.
Don't be put off, it'll take all of 5 mins to complete (if that). It asks whether anyone is claiming or receiving Carer's Allowance for you, whether any other adults are in the household & if so whether they get DLA themselves ... then it's just a signature and into the post it goes.0
