Has anyone got e2save cashback forms?

I really need a copy of the e2save cashback claim form - I've lost mine.
If anyone can help please PM me! (all I need is a scanned copy (as far as I know))

Do you mean the instruction sheet that tells you how to make a claim?
It isn't a form to fill in (as far as I can remember) but merely instructions of what to do. E.g. send your 12th monthly bill with a letter giving your details etc to E2save.
I could scan this for you.

If you bought your phone recently, phone up e2save and tell them you didn't get a claim form. Otherwise, I might have a claim form lying around somewhere, let me know if you still need it.

Thanks everyone, I found it in the FAQ - because the contract was purchased so long ago all they want is the bill - they don't want a form along with it (I would've lost out on £360 if I'd not been able to make my 2 claims!!).
