Can you burn bbq charcoal on an open fire?

Not as daft it sounds, I remember someone died from that, an immigrant family i think, they used disposable BBQ's to cook on and heat the rooms but opened windows, but alas the CO2 got em, very sad.

Would you lease explain to us how charcoal, burning in a grate beneath an open chimney could be more dangerous than an equivalent amount of coal which, in addition to carbon monoxide, releases a plethora of noxious fumes?0
