Transfer from Nationwide to Ruffler

Sorry to start a new thread, but I couldn't find the answer to these questions below anywhere. I've never transferred an ISA before so have got a bit stuck.
I am transferring out of Nationwide after having a Cash ISA (only 5% :rolleyes: ) with them for about 10 years. I've signed up with Ruffler and they've sent me a "Cash ISA transfer request form". I assume this is to be sent to Nationwide, but can't find any details on their website as to where to send it to.

There is the address of the local branch in the passbook, but do I send it here, or to the main Nationwide office somewhere?

    Have you got a letter/certificate from Nationwide when you first took out the ISA or even your yearly confirmation? This usually has the address on it.
