I remember this happening to me when I worked for the NHS.

I was off sick with the flu and after a few days dragged myself into work. I had to go home at lunch and then had another day off. This counted at 2 instances of being off sick and together with another day I had off counted as 3.

After that I was careful to make sure I was completely better before going back to work.

I think that the three people at the meeting would be as follows:

    someone from HR
    his line manager
(maybe a secretary to take notes)

    Or

    someone from HR
    his line manager
    and someone from Occupational Health

    Just a thought.

He isnt being disciplined - its just a meeting to see if he has a problem and whether or not he will need help.

    Hope it all goes well.

God, reading stuff like this reminds me of some of the reasons why I left the world of employment and am now very happily self employed!

That would be trying to treat it like a disciplinary meeting, which it isn't (as far as I can see).

Having a meeting with three people does seem a bit OTT though.

You should NEVER go into any sort of meeting on your own. 3 people is intimidating and you have the right of a "MCkenzies friend" in with you. I always went in someone even if it was just for moral support.

a lot of companies have a sickness policy of this sort - its not unusual and nothing to worry about. in fact i fully expect to have a return to work interview next week and go on to a 3 month absence review, i have had one absence of a month due to recovery from surgery.

A McKenzie Friend is for legal proceedings, not for a sickness absence meeting!

The right to accompaniment is only relevant to grievance and disciplinary meetings. Any right to accompaniment in this situation would be at the discretion of the employer.

Many employers have something in the contract about sickness absence when you join them, I know that major supermarkets do it.

In that case it would not be a McKenzie Friend, and you have no right to take someone with you (in any capacity), that would be at the companies discretion.
