code failed may mean all tries have already been used?
have you had it from new? also some are refurbished/returns which someone may have already tried codes in before

im sure its already unlocked m8
phone voda cust services and ask them if its showing as blocked on there database -if it is phone orange and ask them to sort it as they may have passed the wrong imei to other networks for blocking
