One minor point in your letter - you firstly say that the charges have caused you financial hardship but later on go on to say that you will take steps to clear not only the overdraft but the loan as well!

I had to take a loan to clear my husband's balances as he couldn't afford to!0 -
No such thing as Banking Ombudsman. And you dont take advice from them but complain to the Financial Ombudsman Service.

The charges incurred were unfair and excessive and have now caused me financial hardship. As such, I request a full refund of the total amount of £538.90. If you refuse to refund the charges I will be making an official complaint to the Financial Ombudsman Service.0 -

The only thing that worries me is that, if they disagree on the "financial hardship" status, anyone scanning this letter (as I used to do for a living) will instantly file it under "bank charge reclaiming" and reply with a standard "our charges are fair" letter...
I think your case may be argued as different, the key difference being their failure to contact you. If you can prove that they "noted the address wrongly" (I think someone admitted it during the phonecall..?), and concentrate on that point instead, you might get more attention.

Eek, I'm not sure! It's actually the "unfair and excessive" bit I'm worried about - yes, it is true, but it's also standard argument against bank charges generally.
