Hello,

Please could anyone tell me what details I need to give to CPW in my letter of termination of contract and requesting my PAC.

    You need to send the termination letter to your network not CPW. You just need to put that your giving 30 days notice as of the date on the top of the letter and you request your PAC code, they will usually send this out in letter, some networks do text it to your phone also.
