Halifax to Icesave ISA Transfer Issue

Guys wanting your help advice with this problem. My sister has just opened an Icesave ISA and filled out the forms and sent them to Icesave. A few weeks later she received a letter from Halifax stating that they have closed her ISA and the money had been sent onto Icesave.
She has now received a letter from Icesave stating
"Unfortunately we have been unable to accept the transfer as Halfax never completed the transfer documentation correctly
Your ISA funds have been returned to your current provider and requested again"
Now obviously the first thing I will get her to do is ring Icesave to confirm what Halifax filled out incorrect. Then contact the Halifax ISA transfer department. But I wanting to see if anyone can advise on if they've had any problems like this? how long it will take Halifax to correct? Will they have to be constantly chased? What is the stated time frame that banks say they will complete transfers by, because obviously this is a Halifax issue so any timeframe is from the original date and not this new returned date.

My advice would be avoid Icesave - terrible customer services when I've been tryng to transfer OUT. I originally went from Halifax to them with no problem but I found that they're extremely unhelpful/unresponsive when you try to get your money transferred out!!!!!0
