It's not totally white on top, more a sort of grey. Maybe the soil is getting old!

Might be damping off, top of the compost goes white and fluffy! If its that then your seeds are most likely gone
Too much water and not enough air do it most of the time, try to water seeds from the bottom till they have some true leaves on them 0
