I did check these earlier but noticed that the first link you've posted above doesn't appear to work. Also, the second link isn't an online form, thought it might be OK to post the one I spotted as it's an actual form rather than a phone number or email address.

Thanks, that link works fine.

BTW The form is a *bit* broken;
the address finder popup appears the wrong size and without scrollbars (can be manually resized to find the button concerned)
the captcha text box does not clear on focus (leading to wrong code input if you don't spot that it's still populated)
if you miss a required element (drop down) the form refuses to submit, but there is zero feedback

I mention not to be pedantic, but in case other users are having trouble getting the form to submit.
