how do I get a pet passport?

Hi, my mum is moving to spain in a couple of months and wants to take our boarder collie with her on a pet passport but we have not got a clue about how to do it. All the sites we have visited are not really much help and we don't want to try the vet incase they try conning us in to getting lots of injections and stuff we don't need.
If some one out there has done this with there pet could you please tell you how in some sort of step by step. She is up to date with all her jabs (the dog that is, not my mum) and are just in the process of changing the name of the microchip over to my mum.

Havea word with the vet were you take the dog for the jab they should have upto date information on pet passportsIf you look anything like your passport photo....Your too ill to travel0

I am currently getting my passport sorted for my puppy. My vets have said that he has to have all his vaccinations up to date. Plus he has to have a rabies injection and be microchipped. There are then complications with the date he can leave the country after having their rabies injection. My dog is having his on the 30th September, he then has to return in one week for a blood test. He can then leave the country, but cannot return for 6 months. After that period he can come and go just like us.

You only need a pet passport if you are bringing your pet INTO the UK, so unless Spain has similar rules you don't need a passport.
However if you mother is ging to return to the UK at some point I would have thought the information in
