Lloyds Account monthly fee

My OH has just cancelled this account as it has been costing him every month.
He never remembers agreeing to it nor has he used any of the so called services. We have been paying for breakdown cover and holiday insurance via direct debit for years and so these so called free services for a £15 monthy charge were never needed.
I believe as does my OH that he was dupped into signing for this account when he renegotiated his OD facility or took out a personal loan.
He has been paying it for years!
We have spoken to the bank and asked for a refund in full but they are sticking there fingers up?
Do I go to small claims or the ombudsman for miss selling of a finantial product.
OH always thought the account fee was a charge for going OD and we fully intend to claim those fees back also.

Probably not. Unless I'm mistaken account fees are one of the few things you can't claim back0
