Boots Viewpoint is giving away 11 prizes to people who join this month, including a grand prize of 100,000 Boots Advantage card points (worth £1,000) and 10 prizes of 5,000 points (each worth £50).

To be entered into the prize draw, all you have to do is to sign-up for Boots Viewpoint by filling in our questionnaire - it should take no more than 10 minutes to complete.
A full list of the terms and conditions for the prize draw are:

• Prize draws are open to respondents who complete the sign-up questionnaires prior 1st June 2008.
• Only one submission per email address will be entered into the draw.
• The winners will be chosen at random from all eligible entrants on Monday 2nd June.
• The winner will be notified by email. Delivery of the prize will be within 4 weeks of the email.
• The prize consists of one first prize of 100,000 Boots Advantage Card Points and 10 second prizes of 5,000 Boots Advantage Card Points. The points will be credited to the Advantage Card Number provided in the sign-up questionnaire.
