Can wife reclaim??

The wife opened a basic bank account with HSBC to get her Incapacity benefit paid into, this was great she had a solo card and couldn't overdraw by more than £10 just to give her access to funds less than £10 in her account.
She was later awarded DLA whuch goes in a savings account, when she got this HSBC upgraded her to a full account with £100 overdraft without her asking for it.
Because of her mental problems, she sometimes withdraws money 2-3 times a day and can't remember what she spends it on.
They have allowed her to exceed the overdraft limit and charged her £17 arrangement fee last month and just notified her they will take £49 this month.
How are they allowed to do this? I have a separate account that all the bill get paid from because she would just spend all our money when we had a joint account.
We can't afford these fees as we are on a DMP after I was made redundant and took a large drop in earnings.
So to recap
They upgraded the account without asking, they set up the overdraft without it be asked for, they allowed her to go over it, all cash withdrawals no card payments DD's or SO's. Now they are taking £17 + £49 from her benefits.
Surely this can't be allowed?

Can you go into the bank, explain and ask them to put it back as it was?
Most banks seem to reverse the first charge if you ask.0
