Witnessed Fender-bender

Coming home from town today, I was driving behind an ice cream van when a polo with a trailer full of rubble pulled out from the right hand side of the road so I let him in (He didn't give me much in the way of choice and the traffic was stop/go anyway).
I could see the guy was pre-occupied with tuning the radio, fiddling with the glove box etc. and so around 1 minute (and 50 yards) later he went into the back of the ice cream van. They got out and looked at the damage and then ulled over to the side of the road. The IC van was largely protected by a tow hitch but the polo lost it's licence plate and dented it's bumper.
From my point of view, the polo was clearly in the wrong. If he'd been paying attention, he could have easily stopped. Besides which, e drove into the back of a stationary van, that's usually enough in itself.
However, due to all the traffic and the fact my daughter was in the car, I didn't stop to give them my name. Should I have for such a minor incident? If so, what do I do now to give my name?

It's usually clear cut. If someone goes into the back of someone else, they are at fault. This is why you get insurance scammers driving around cutting the brake lights and slamming on the brakes to get people to go into the back of them, then suddenly there's a claim against your insurance for the car and whiplash for 4 people.0
