maternity allowance

anyone know the address i send my maternity allowance form to.it tells me to look up the yellow pages but i cannot for the life of me see it:o. it says to look under Jobcentre Plus but i cannot find it.any help would be greatly appreciated.

It will depend on where you live.
In the BT phone book look under under either 'Social Security' or 'Jobcentreplus'.
In the Yellow Pages, I think you need to look under 'Government'.0 -
