Sky Credit Card Please Help

I have a sky Credit card that I got into debt with and now the debt is with CapQuest. I can't find an old Sky credit card statement. I have rang CapQuest and they have given me my Sky Credit card number but I can't find an address anywhere for Sky Credit card to send and request a list of my Charges.
Has anyone requested charges back from Sky credit card before and can anyone tell me what address I need to send a request for a list of charges to.
