travel insurance confusion

i just got single trip cover for two weeks to go to france (tomorrow! last minute deal) thru simple as recommended by this site... it was a good price and all went thru okay.. but im cunfused as i got a conformation email from them with two pdf files attached.. they seem to be forms i need to fill in myself but i dont know what to write in have the boxes... should they not be sending me specific documents to me and my cover already filled in?

im a little worried about it so would appriciate some advice

EDIT: not sure why but it seems that when i was looking at the attached pdf's as attachements to the email in outlook there wasnt any specific info to me on the documents but when i actually open them in acrobat the info is all there!

not sure why...
but scare over!
