moving house+mortgage

hi i am about to put my house on the market and start looking for another, my question is with my mortgage, it says its 'portable' what does this mean can i just move house and mortgage or do i still need to re-apply for the same deal but on the new house.
since buying our current house my wife has left work to be a fulltime mum so we have lost an income although i can easily afford the bills, when it comes to the bank if we need to re-apply then my wages wont be enough(by there standards anyway) for the new mortgage,
i was hoping they would just keep my mortgage account and just change the details, im not borrowing more or less because the new house is similar in price and any proffits would go into the new house to keep everything the same.
an i going to end up in the doo doo with the wages issue..????
