LEGISLATION OF EVIDENCE: Forms DBD370(N) GPFR and DBD365 Generic

I hope someone can help.
I have claimed DLA and am going to appeal as I have been refused. I have received a consultants report, that is not in my favour, so would like to ask the following:
What is the legislation with and the difference between “DBD370(N) GPFR” and “DBD365 Generic”, General Practitioners report and “hospital/consultant report” respectively?
Are both of this reports supposed to be factual and not ones opinion?
What action or process can be taken if personal opinions are stated in the “DBD365 Generic” and/or if it is inaccurate?
Where can these forms or guidance notes be found or downloaded?
Where can I find the legislation on evidence/reports being used correctly at a hearing?
I have carried out all sorts of searches, but can not find the appropriate information.

The DLA Decision Maker's Guide is of more use to you, rather than the DLA law and is here-

Select - Vol 10 Chapter 61
The DBD forms you mention are just that- forms that the DWP use to collect factual medical opinions on to help the DWP Decision Maker (DM) decide the DLA claim.
If you have doubts on the evidence used by the DLA DM, you will be able to question it at the appeal hearing.0
