Orange Broadband - managing directors address?

Hi, my cousin is being threatened by Orange for money they claim she owes. They have given her until April 11th to pay it.
It all started in April 2007 when she asked to have her contract cancelled and for the puk (?) code. It took Orange until September 2007!!! to finally provide Sky broadband with the puk number. She then didn't take any chances and didn't cancel her direct debit with Orange until a month later to allow for the changeover. She has a letter from Orange confirming she is leaving, dated September '07. She also has a bank statement dated October '07 for the last Orange payment.
Orange are now saying she owes them just over £40. She can't get any sense out of the customer services whatsoever despite having proof that if anything she over paid. Not to mention they took a staggering amount of time to release her puk number to Sky, during which time of course she couldn't do anything other than stick with Orange.
So, does anyone have a contact address for anyone "high up" as it were? because as you can obviously tell, this debacle has gone on for far too long and gone a step too far now with demands for money.
